Vertex (0,0)
Focus (0,6)

You should recognize this information to be sufficient to suggest

\(4py = x^{2}\), where \(p\) is not yet known.

If you REALLY remember what will fairly likely be required of you an an exam, you should also notice that \(p = 6\).

Almost an eyeball problem if you remember the general form and the complete meaning of the distance from the Vertex to the Focus.
