It would end up like the big cities way back in the old day (when they still used horses for travel) People, and animals would get sick and you would have trash in the gutter of the street and it would look really gross.

Yes that's true because they didn't have modern medicine, but the waste and trash in the street only sped up the process of them getting sick.
