i think it is safe to say that the blame was shared. the point shakespeare was trying to make was that it was the feud between the montagues and capulets that was to blame, not a specific person. their parents were to blame because the feud forced the lovers to hide the relationship, the nurse and friar helped by allowing the relationship to bloom further, the banishment for tearing the lovers from eachother, and of course the lovers themselves are to blame as well, not only for taking their own lives, but also for defying their parents and ignoring the feud.
