A is wrong, many times were codes used. The Zimmerman notes are just one example of an intercepted message that was "coded." B is wrong, codes were useful until they were cracked, then you had to generate a new one. C is true, a lot of times commanders used codes to communicate. And C was wrong, Zimmerman notes and the recovery of the enigma system caused codes to get cracked.
