Can someone please give me an example of classical conditioning from personal experience? If you do, please state the unconditioned stimulus and response, controlled stimulus and response. Thanks.

People working for me were not signing their pay receipts. I added a hand-written arrow pointing to the place where they were to sign. They signed, either to keep me happy or not to seem dumb.

I am not that familiar with your terminology. The unconditioned situation was the routine pay stub, which they were supposed to sign but often did no. The new condition was the stub with a large, hand-ddrawn arrow pointing to the place where it was to be signed. To have my approval or not to feel stupid, they signed routinely ever after, and they conditioned me to keep adding the arrows! You can probably make this fit your assignment.
