How do you organize the top of a school essay?

There are supposed to be the:

Date, Teacher, Lesson and Unit, and Your Name. I just don't know in which order to put them.

Well, there isn't a certain way to put it, but I would organize it like:
Date
Lesson and Unit
Teacher
Name
