"Silence!" the judge shouted, "Or I'll have you removed from the courtroom." See how there's no need for further capitalization? There are no periods/question marks. You do not capitalize after commas unless you are saying a persons name, business, book, and so on. You also need a comma often before starting quotes. So therefore D is correct. There is no period, there is no question. Hope this helped :)
