Why did Justinian I fail to regain all of the former provinces of the old Roman Empire?

The short answer is that his forces were already stretched to the breaking point and that there wasn't enough money to secure much of what was gained. By the time Justinian's reign ended, the army actually reduced in size because the empire literally didn't have enough money to pay for the soldiers needed to hold onto all of its new holdings.
