we can stop this by getting better moderator tools to deal with it.

I have also seen this happen when the lag is bad. So if you get that many notifications but there is only one tag, it is a random event and nothing can be done at the moment. However, if you go and there are lots of tags or you can watch them tag and delete, then it is a moderation issue and should be reported.
