I think context is important here. But without it you could probably figure it out. He has sadness (the grief) that lies heavy in his breast (his heart). AKA he's heartbroken. Romeo is saying how he is heartbroken over a girl that doesn't love him back. He explains this when he's having a conversation with Benvolio early in Act 1. So I believe the answer C
