What can happen when a fission chain reaction gets out of control?

You mean in a situation like a nuclear reactor? Out of control means that it produces more heat than we can remove from it. The temperature rises and eventually the materials making up the reactor start to melt. That's why we call this situation a melt down. If then the superheated fuel comes in direct contact with the water you can even have an explosive reaction like in Fukushima.
