Juan Carlos se lastimaba cuando jugaba basquetbol. It is imperfect preterite.
Juan Carlos se estaba lastimando mientras jugaba basquetbol. It is past progressive using imperfect preterite estaba+ gerund lastimando. Jugaba is imperfect preterite too..
Juan Carlos se estuvo lastimando mientras jugaba basquetbol. It is past progressive using perfect preterite estuvo+ gerund lastimando. Jugaba is imperfect preterite too..

@Bertide23, @vain you can use mientras (while) or cuando (when) with the sentences.
