you're basically diluting everything. 

Is the 250 mL for all the ingredients together, or each one individually?

250 is the total volume

So just use that dilution formula, for example:

Aminosyn 2.5%              Aminosyn 8.5% 500 mL

$\sf (2.5\%)*(250~mL)=(8.5\%)*(V_2)\rightarrow V_2=73.5294~mL\approx 73.53 ~mL$

Ped MVI 5 mL                   Ped MVI 10 mL

I assume that the container has 10 mL and you need 5mL, so just add 5 mL of it.
