How often do you need to evaluate your exercise progress and make changes to your exercise regimen?

I would say every 6 weeks or so for evaluating your progress, as for changing it around. Do that only as needed, if you feel like you have plateaued then switch it up.

I'd say it's whenever your body is used to the workout. When the workout becomes easy to do, change it.

OpenStudy (anonymous):

That's what plateauing means. Whenever you stop making your muscles sore from that particular workout.
