How do you find the surface area of a non-linear function around a line?

OpenStudy (anonymous):

(I can find the surface area of a function rotated around both the x- and y- axis, but I don't know what to do with the line.. possibly split into x and y components? But that's extremely complicated for some functions. Possibly in the integral, instead of finding the radius with relation to the x- or y- axis, find it in relation to the line? But what about the effect of the slope of the line of the function being rotated?)
