I think this will solve the solution of blank reports, and especially when users are reporting a user in the chats without stating the name of the chat subject. We can have something similar to this design of the Report Abuse feature.

the report should already know where the report is being activated from so requiring a field for it is not all that requiredable.

chat reports are some of the most obnoxious reports that a mod gets simply becuase the link sends us to some page that we cannot use our mod tools on.  we then have to hunt down where the incident occured in order to assess the situation.
