Hi Taylev :)
There are some criteria with which moderators are chosen here ...
1. Most important is that the user must follow (and inspire others to follow) the code of conduct of OpenStudy.
2. Expert in any one of the subjects. 
3. Should be patient and mature enough to handle variety of situations
4. Never be rude or get irritated and start cussing.. 

If you satisfy all these, you just wait for an administrator (or other moderators) to recognize your potential..... there is no form for application ....one day an admin will contact you to ask you whether you wanna become a moderator....when i was asked, i agreed :)
