You do not lose your any medals when you buy a title but you will lose your smartcents. And you get 1 smartcent for each 1 medal you get! And don't worry! You can never go negative in smartcents because it isn't possible!

And getting even the lowest title is helpful because people know you are good in that subject and might tag you more!
