I have a slight concern about a part of the CoC

People see this in the CoC -

$\rm \color{blueviolet}{\text{If people are going to spend time to help you,}}$ 
$\rm \color{blueviolet}{\text{you should at least spend the time to ask your question as clearly as possible.}}$

This is what people always ask me about, because loads and loads of users still copy and paste some questions. They ask me if he/she should report that user. I usually respond no, because almost everyone does it

It just confuses me sometimes, because how can we really stop this?

Yes, that's against the code. I usually remind this whenever I find someone just copy pasting there questions or posting screenshots of their questions.

It's better if they post their thoughts along with the question too and be as much specific as they can while asking the question.

Screenshots are okay if they want to show some diagram or anything like that.
