His attempted reconquest of the territories formerly held by the Western Roman Empire put Byzantium into massive debt. Through years of war and siege culminating in horrible plague, he terminally weakened Italy. The ostrgothic kingdom of Italy, which had largely preserved Roman law and civilization, was unable to stave off the later Lombard Invasions. Thus Justinian was indirectly responsible for the dark ages in Italy. The drain on the treasury also weakened the Empire to the upcoming threat of the Arab invasions. I guess you could call these "accomplishments"....
