You are correct. Since smoking goes into your lungs because you inhale it. This is what causes lung cancer. So the respiratory system(Air intake system) is what would be affected.

Actually it would be both B and C. C is the logical choice since there are direct damage to the alveoli through smoking. It causes morphological changes in the lungs with prolonged smoking. However, B is a downstream effect, since cigarettes contain nicotine (and other components) which raise blood pressure (by constricting blood vessels) can over prolonged periods make a person more prone to serious heart conditions and dangerous long-term hypertension.
