Pretty simple. "issued a warning" means someone issued you a warning. From what I have heard, a warning serves as an official reminder for you, asking you not to break some rules.

Your IP however is kept by OS, and they can see how many accounts you actually have on your IP.
So if you find away to make a new account, I'm pretty sure they'll find you LOL.

when a warning is issued to your account, you're breaking a rule (like when you post test questions).

my suggestion is that if you want to break rules, make a new account and remember to use one of those proxies so that they can't see your IP. good luck!
