I'm pretty sure it's B, how things are said.
The reason I believe that it's this is simple. Every writer has their own unique style, making writers like J.K. Rowling and J.R.R Tolkien have their own way of pulling you into their worlds.
So it's specifically HOW it's said, not when, not what, and not where.
