I am stuck on either D or A

It is D, because for A, it would have been "is" not "are"

No, I respectfully disagree. If you read D out loud, it is not correct. When speaking in plural terms, "Nurses" would only need "appear" not "appears". It is B. Tornado is singular and "is" goes with it.
