I would go with B. The question mark is inside the quotation marks. And there should be a period at the very end, which B does.

Not in all cases, see "she" is not capitalized in any of the answers.

But I can see the confusion. The key here is that the question that is being asked should be enclosed in quotation marks. It is a question, so there needs to be a question mark inside of that quotation. However, "she wondered" is not asking a question. It is just the part of dialogue that describes how the dialogue is being said. Does that help a little?
