It's the total amount of fans that you have either from people that have blocked you, unfanned you, or the opposite. So yes you have lost fans and your notification is going based on the total amount as if those people that unfanned you or you have unfanned were still there.

see I unfanned you ^_^ so that makes it one less in your accurate list and one more in your fan notification list.
