all dates in the USA are written as: EX: May 28, 2015 The comma goes between the Day and the Year.

That takes care of A and B, but what about C and D?
It is C. when you list dates, a semi color goes in between each date. EX: June 14, 1998; July 17, 2001; November 2, 1986.
