MIT 6.01SC Introduction to Electrical Engineering and Computer Science I 12 Online

Course notes Spring 2011 of 6.01SC. Chapter 4 (about state machines). I can't make my accumulator work. I typed everything like in these notes but it doesn't work. Please explain me how can I make examples on page 125 of these notes, work?
