A mutation is a change in the nucleotide sequence of a cell's DNA. They can result from the insertion, deletion, or substitution of one or a few nucleotides in a gene sequence, or even rearrangements of entire genes within a chromosome. These changes are often caused by errors in DNA replication prior to cell division, resulting in daughter cells that are not genetically identical. Mutations can be harmful, neutral, or beneficial to the organism. Cells that contain the mutated, or changed, DNA will pass on those mutations to daughter cells through binary fission.

although mutations occur in the areas you pointed out like deletion, insertion or substitution, even one mechanism can provide one or more of the three.
