I think your right, because when we talk about adjective phrases, it has to modify the noun... So I think that C. is the right answer.

I was thinking A... well-done describes how you would like your hamburger. 'Without' describes a new noun in the sentence, cheese. Hamburger is the main noun in the sentence. I may be incorrect though.
