Should a president die in office, which official succeeds the president if the vice president is unable to assume the role?

If a president dies in office, and the vice president is unable to assume the role, the Speaker of the House is the next in succession. If the Speaker of the House is unable, then it would be the President pro tempore of the Senate, followed by the Secretary of State... and so on.
