How would you ask your teacher her nationality in Spanish?

There are a few ways to accomplish this task. 1. Ask them where they are from using the formal Ud. "De donde es Ud?" 2. Ask them their nationality "Cual es su nacionalidad" where "su" is the formal "your". 3. Ask them where they were born - "Donde nacio Ud?

or you can ask then "De donde es?"

maestro1631 have answered you well, only a few things about punctuacion and grammar: remember in Spanish there are two questions marks: to open and to close the sentence: ¿De dónde es usted? or ¿De dónde eres tú? At the same time, remember to put a tilde over "dónde" when you use this word to ask. And nació has a tilde too.
