so basically they want me to use the planning process to plan out last year of high school (grade 12).

My future plan is to get into a University for Computer Science my choice is York University, it requires more than a 70% marks to get in. The first way to accomplish this goal is to get higher marks in my all course so that I can easily apply for York University. The second way to accomplish my goal is to really work hard for achieving it. I believe that I am not very far related to the mark but the main thing is to maintain it. I think I am not that far from my objectives but still need to work on my weaknesses because sometimes weakness is what causes and may trouble. I believe that right know I am at a stage where the things might go right and may even go wrong. I have to keep focus on my plan and try to achieve my goals because it would affect my future. There are so many things in life that are suddenly happen which may also affect our future plan as well so that why we have to care about those things such as the example of this be if I wrote a test at the end of the last semester and I fail it that would affect whether my marks.

Now, for the second one, all it is asking is for some examples of things you have done that you believe are contributing towards you reaching that goal. It also asks you what you think your strengths and weaknesses are toward reaching that goal. Also, I am currently a Senior in high school, so I suppose I could be of help here. :p

Also, yes, I am sure about #5. If you look at the title above it, it says to implement the plan, which means to put that into effect. It's telling you to follow your plan so that you can reach your goal. But it also says that if you encounter a problem, you can change/revise your plan to better suit the situation(s) at hand when the time comes.
