DNA forms chromosomes in cellular division processes such as meiotic division and mitosis. It happens in prophase (in mitosis) and prophase I (meiosis). What happens is the condensation of the DNA leading to this particular organisation. Basically the DNA winds around proteins and folds into chromossomes.
