Using y=mx+c, x and y will be a point and m will be the gradient. Replace all the points in the equation to find c. After that, rewrite the equation putting values of m and c only. Can you do that?

m is change in y over change in x.\[m= \frac{ \Delta y }{ \Delta x }=\frac{ 3400-1000 }{ 2012-2000 }=200\] One of the points to use can be the (2012;3400) one.\[3400=200*2012+c \rightarrow c=-399000\]\[y=200x-399000\]
