well to start, this is one of the first songs in the metal/punk genre to openly deal with self harm. it is also, according to Randy Biersack, about how knives or pens can be destruction or creation, respectively. it could also be a testement to stop self harm and create somthing, like music or art. hope this helps. a lot of people have been asking this question

Ahhhh, bvb <3 But I heard that the knives stands for destruction, as in cutting yourself and the pen stands for creation. You have the choice to choose whether or not your life is gonna go your way or have someone else control it. As for Andy, the pen stands for his choice of deciding to make music and making something out of his life. Soooo, kinda saying by making good choices in life, you can overcome the troubles you come across. It's all by choice though.
