no worries, i did horribly in english class, i think it's just being interested in the material what drives you will to learn. Science is just process of how to go about collecting knowledge about the natural world, so it's super interesting... anyway. 

Hearing, in short, is caused by vibrations of tiny hairs in the inside of your ear, each one of these hairs is "tuned" to a frequency, these hairs relay the message through other systems to your brain (this explanation can be really long, if you're interested in the long answer, google it or look it up on youtube.

Explain how the data allowed you to form information.

you can talk about the symbols the sounds represent, forming a word.

Explain how you combined that information with even more information to form knowledge.

Talk about language, how you gave meaning to the word you heard.

When I heard what JEssica's names was; it caused by vibrations of tiny hairs inside of my ear, ewhich relayed the message through other systems in my brain.

This allowed me to connect things like how her laugh sounded to her name; among other things.

This also allowed me to connect how her room smelled; how she looked and what her voice sounded to her name.

I mean, i don't know what the teacher is expecting, but i would talk about the data itself.

Each syllable in the persons name is the data you heard, you were able to put together those syllables to form a name.
