However hard he threw himself onto his right, he always rolled back to where he was. He must have tried it a hundred times, shut his eyes so that he wouldn't have to look at the floundering legs, and only stopped when he began to feel a mild, dull pain there that he had never felt before.

feels weak and faint Does anything support this? Is he weak, or just uncomfortable. Bring out evidence.

And he never turned into a bug. That is so far fetched, and obviously not correct.

Again, he hinted HIS LEGS, so, it might be him, in the bed, after an accident, regretting how his legs look now.

However, I did look again in the paragraph, and the title summary.
His dream is saying that he is imagining himself turn into an insect, though it seems that his legs are in the result of a injury. So, it could be conveyed as both, but I still think C. (It seems more likely than the dream XD)
