How do I find and put the slope?

I think you mean: "Find the equation of the straight line with slope 1/4 and passing thru the point (0,-4).
The slope is given in the problem statement.
Please plot the point (0,-4) now.

Next, draw through that point a line with slope 1/4. Note that a slope of 1/4 points to a "run" of 4 and a "rise" of 1.
