The answer is B. "Uncle" is capitalized because it is a part of the name "Uncle Bill". Doctor doesn't have to be capitalized in the sentence because it is just a noun. For example, if I was a doctor, I would be addressed as Dr. Morgan. I hope this makes sense. :)
