D is the one that is incorrect. The sentence should be: At first the twins, Lucy and Edward, thought we were imagining things.
That would be wrong...It would be the one that doesn't sound right when you say it out-loud. Try saying all of them out loud how they are already punctuated and see which one sounds incorrect, but it is not D like @dawnflower09 said, that is correctly punctutated
