I feel so cold
I feel so old,
laying in bed
pillow under head
people around me,
love surrounds me
mere hours have past
 eyes drooping fast,
take a hand make the moment
         last,
tears run down faces
regret acts as braces
tied together like laces,
  all in one room
will be grieving soon,
 I can't hold on
    no longer
she says to me,
knees go weak
cannot speak
breathe in breath deep
keep it together I say to
          me,
look up you're smiling at me
" it's okay you can weep".

It's about someone dying (probably an older person). It seems to switch POV towards the end.

@ladycielphantomhive you are correct it switches POV because the person dying has past and the other person is trying to keep it together  and break down in front of the other family members, but then they look up and see that the now deceased relative is standing in front of them smiling letting them know that everything will be okay and thy don't have to be strong anymore.
