Both #1 and #2 are asking where the person (student/teacher) is from.
"From" in French is indicated by the preposition "de", example, "Je suis de Toulouse".
For #3, I suggest you go through the pronounciation dictionaries (ex.
http://forvo.com/)
and learn the proper pronounciations instead of guessing.
#4 is correct, as AloneS indicated: an exercise book is indeed a classroom supply.
