You're correct except for the banana. Bananas turn brown because sugars (or something, lol) oxidize in air, or because they make ethane when they are ripening and the reaction keeps going (answers depending on whether or not you've opened the banana).

@SapphireMoon so is #4 right or wrong, should it be both physical and chemical.

4 is wrong, @Firejay5. It should be a chemical change (oxidation).
