It seems that question one had already been answered.

As for the things that could have been done to prevent Phil's death:
- They could've had their activities under supervision of a lifeguard
- The friends could've called Phil over when he was surfing too far out
- Phil could have refrained from drinking that day
- Called a lifeguard/911 and report him missing

As for the last question, I think it's more targeted at situations of peer pressure, and what's deemed as 'cool' to do amongst young people, e.g.
- Someone taking excessive amounts of drugs, should you have stopped them?
- Someone trespassing land
- Allowing something to take place, e.g. bullying

However, this is an account of personal experience, so I can't write the answer for you, but hopefully I gave you some ideas.
