Equations of planes come in several forms. Have you looked up "equation of plane?" Were you to Google that, I'm sure you'd find several forms. Perhaps one of them would require that you provide the coordinates of the 3 intercepts of this plane with the x-, y- and z-axes. Alternatively, you could form 2 vectors in the plane by connecting 2 points at a time. Cross multiplying these 2 vectors would give you a vector which is perpendicular to the plane. With that, and using ONE of the 3 vertices, you could then write the equation of the plane.
