Standard form is y=ax^2+bx+c

When focus and directrix are involved, form is y=4px^2 when the vertex is at (0,0)

No.  I'm asking that you type out an equation that applies to situations where the vertex (coordinates of the vertex) is given you.

It looks like y=a(x-h)^2+k.  You really do need to know this.

(h,k) represents the vertex.
