C. Requirements Creep AKA Scope Creep. This is nearly universal in software projects, as the work is being developed and tested the client uncovers all of the exceptions they need it to process. This example is really out of scope because it is really a new unrelated project. One way to combat Scope Creep is to look a lot of examples of the current records, spreadsheets, notes, etc... . You will find many of the exceptional items that will come up recorded in a notes column or in the margins of the documents. These are often critical exceptions that need to be allowed for in the original specification.
