Well the content should be appropriate (essentially on topic). Spelling is important, as well as organization (you don't want things to be all over the place). D makes sense because as long as your diction gets the meaning across, then it's fine. It is not necessary to further 'enhance the content's meaning'

It is D. "The word choice must be exact" doesn't even make sense. And enhancing the meaning is more so for creative writing. In general, as long as the meaning is getting across, you're fine.

It's asking for the "least important factor to consider when revising your writing." D is hardly as importance as A, B, and C.
