To me, the example looks like a quick summary and some information to find the source. It doesn't try to cram a ton of information onto the note card. That's for the research paper :)

Not exactly. D sounds like the Work Cited page, not note cards used for gathering useful bits of information on sources. The answer is A because it's not about putting a ton of information or a direct quote. Note cards is a simple method of finding an interesting source, writing down useful information, then a page number or some way to find that source again.
